of the order form. The third line of this section is entitled "Name
on Card" Please enter the name as shown on the payment card.
NAME ON PAYMENT CARD > EXACT NAME
Please enter your name or the exact name as shown on the payment card
in the box provided.
NAME ON PAYMENT CARD > PERSONS TITLE
If your name on the card does not show a title for please add this at
the beginning of your name for our benefit. Please remember this information
is important to us for our records and to the card company. All information
remains confidential.
NAME ON PAYMENT CARD > CORRECT NAME
Please enter only the name of the actual cardholder even if it is not
the same as the customer name. The card details are for billing purposes
only and separate from the order details.
NAME ON PAYMENT CARD > NAME AS ON CARD
Your name should be entered as it is shown on the card even if you consider
the details on the card to be incorrect or mis spelt. You may add an extra
note pointing out a mistake on your card in the "extra information"
box if you wish.
